What is Medication Therapy Management (MTM)?

The MTM program is a series of personalized consultations, provided at no cost to our qualified patients, with a certified health care professional or licensed pharmacist regarding their medication regimen. The MTM program is designed to ensure that covered Part D Drugs prescribed are appropriately used to optimize therapeutic outcomes through improved  use and reduced adverse drug interactions.  FreedoMed Pharmacy is excited to offer our MTM program, designed to meet the special needs of our   Medicare patients with multiple chronic diseases and multiple prescription medications.

FreedoMed Pharmacy's philosophy is to improve our patient's health by teaching them how to understand and optimize their medication therapy. Through personalized consultations with a qualified Pharmacist, we can help achieve this goal.

What occurs during an MTM Consultation?

To be eligible for MTM services, patients should meet the following criteria:

  • Perform an in-depth assessment of the patient's health status.
  • Formulate or modify an effective medication treatment plan to identify, resolve and prevent medication-related problems, including adverse drug interactions.
  • Administer the medication therapy.
  • Monitor and evaluate the safety and effectiveness of the treatment plan.
  • Document care delivered and communicate the essential information to the patient's providers.
  • Provide verbal education and training designed to enhance member's understanding of the appropriate use of his/her medications, thereby enhancing patient adherence with his/her therapeutic regimens.
  • Coordinate and integrate medication therapy management services within the broader health care-management services being provided to the patient.

  • Enhance patient understanding of their prescription medications.
  • Enhance patient compliance with their prescribed medication therapy.
  • Detect adverse drug interactions and patterns of over-utilization or under-utilization of patient's prescription medications.
  • Detect and avoid drugs deemed to be inappropriate for prescriptions to the elderly, which can contribute to common geriatric problems such as confusion, constipation, falls and other conditions.
  • Learn of lower cost alternatives.
  • Optimize the therapeutic benefits of our patient's prescription medications.
  • Provides a detailed overview of the patient's medications to their physician with suggested changes to the patient's medication regimen.

Am I eligible for MTM services?

  • Patient must have at least one chronic medical condition based on recent prescription claims.
  • Patient is filling more than 5 medications a month.
  • Patient needs assistance in medication management.

How do I Register?

To register for MTM services, simply complete and return the enclosed postage-paid Business Reply Card. Patients will be contacted by a qualified health care professional to discuss the medication therapy management program. A phone consultation will be scheduled with a licensed  Pharmacist to begin the process.

The MTM program is good for the entire calendar year, and patients are re-evaluated for participation on an annual basis.

This program will help patient compliance with their medications. It's convenient, free and provides fast personalized pharmacy service to the patients in need of special attention.
